Gynaecomastia—the enlargement of male breast tissue—is a common yet often under-discussed condition that affects a significant number of men. It's estimated that up to 60–70% of adolescent boys will experience some degree of breast enlargement during puberty, and for approximately 30% of adult men, the condition persists well beyond those years. While not medically dangerous in most cases, gynaecomastia can lead to emotional discomfort, social anxiety, and reduced confidence—especially when it comes to wearing fitted clothing or participating in activities like swimming or exercise.
The causes of gynaecomastia are varied. In some cases, it can be traced to hormonal imbalances, where there is an increase in estrogen relative to testosterone. Other contributing factors include certain medications, such as steroids or anti-androgens, chronic health conditions like liver disease, or lifestyle influences such as alcohol and recreational drug use. Obesity can also play a role, as excess fat increases the conversion of testosterone to estrogen, although true gynaecomastia is distinct from fat-related chest fullness.
Patients often present with firm tissue beneath the nipple, sometimes with tenderness or a visible fullness in the chest area. For those in whom the condition does not resolve on its own—or who experience significant psychological or functional discomfort—surgical treatment is the most reliable option.

Recovery is generally straightforward. Patients wear a compression garment for three to four weeks to reduce swelling and help the skin retract. Absorbable sutures are used, so no removal is necessary. Pain is usually mild and well managed with simple analgesia, and we prescribe a short course of antibiotics as a preventive measure. The excised gland is routinely sent for pathology to rule out any rare abnormalities. Risks from this surgery is rare but may include infection, seroma, nipple loss, asymmetry or residual tissue.
Most patients are able to return to work within a few days, particularly if their role is desk-based. Gentle walking is encouraged as early as the day after surgery, with light cardiovascular exercise resuming by the second week. However, upper body weights, particularly bench presses, should be avoided for at least 4–6 weeks to ensure proper healing and optimal results.
